Usually the middleman trades work in a loop, to prevent things moving between too many owners.
We have person A, with item 1, and person B, with item 2, and the MM.
Person A gives 1 to the MM, the MM confirms it and tells B to send person A item 2. After that, the MM sends person B item 1.
This way, nobody ever holds all the cards.
