Help me make a Form?

I need some help, I am trying to make a form such as a FLP for my forum site.

I took out the original registration and I need to replace it with my own personal form that will email me the info that they input.

Please tell me how to make a form.

Yeah, Jotform just deletes them after a few minutes for this.
