Well I made my own script a few days ago, it is a custom one + fully undetectable. It took me about 5 hours to make it and secure it, so this will not be sent out for free.

I will be accepting requests for custom phishers, remember, if you are going to host this yourself, the host MUST have a database server (MySQL). If you dont want to give me access to configure the database, then I will make you 2 extra scripts which will create database + needed tables. The only thing you gotta do then, is replace username, password, database and table name in the dbconfig.inc.php file and its ready.
