Once an account is banned you can not get it back.
You can get it back if you hack into a mod's/admin's account though.

I've hacked a developer, which allowed me into ModDog with full permissions.
Even if you do hack a staff members account, chances are that you'll get caught and everything you did will be reversed.
If you ever get into a staffs account the best thing to duld be start backing up all the pages of the panel to your drive to look for exploits after.
Then search for an Older [NPC] account that you havn't seen in an announcement for a long while, then use that.
Changing the password to a staffs account will get you caught was faster.
: On Topic, to date there is no exploit or method that allows you into a banned account, or to un-ban it.

You're now required to enter a secondary password before signing in to moddog, which would render XiRaX's suggestion pretty useless.

There is, depending on the permissions of the account in question. Different levels of moderators have different permissions (i.e. I believe omni moderators have the ability to ban users, while forum moderators do not).
I believe there's also some form of IP check these days. If it detects a change in IP, it sends a verification message to the email of the account on file.

All staff are required to login through their IP. At least that's what I've been told. If they log into their account normally it does all the IP Checks, etc. I'm assuming if they log in at a different location with a different IP it throws question only them and gaia know.

The IP check is old. Once you login and the system notice an IP change.. It will email a link to the account where you have to click it in order to continue under the new IP. As everyone know, even if "hack" the mod's first password. Their secondary is always something different. I think GAIA require them to change it every other week or something..
The new thing is that they are offering that option to regular members. (Not secondary but IP thing.)

yes. You can get into old accounts. It involves lots of patience, and only few people know about it. I'm pretty sure it's how google gets his accounts though.

Omni moderators have the ability to ban yes, But that is Not run through moddog, That is run through the APC itself.
They only use an IP check for less trusted staff members, And for really high up server manipulation such as editing currently existing NPC`s or certain features.
